NOTES:

1. When choosing a password, please make sure it is at least 7 characters and a mix of letters and numbers. For security purposes, please also choose a password that you will use only on this site and one that is not used for access to other personal and sensitive accounts (credit cards, bank, web vendors).

2. The state/province field is required to be completed if the country is the USA. If not, you may complete, as applicable and appropriate for your country.
